Brain Scan ImagesThe FDA is warning that patients who undergo MRI and other medical procedures which use gadolinium-based contrast agents (GBCAs) about an increased risk of gadolinium retention, a condition that causes severe bone pain, subcutaneous tissue pain, and “chemo brain.”

What’s the Problem?

There are a large number of potential side effects that may occur with gadolinium-based contrast agents (GBCAs) which may lead to toxicities and gadolinium deposition disease (GDD) and/or gadolinium storage condition, according to the Multiple Sclerosis Journal. Recent cases of toxicities included gadolinium-induced encephalopathy, renal failure, acute pancreatitis, and severe necrotizing pancreatitis have been reported in patients who undergo MRI with gadolinium contrast agents.

Gadolinium Brand Names

  • Ablavar (gadofosveset trisodium)
  • Magnevist (gadopentetate dimeglumine)
  • Omniscan (gadodiamide)
  • OptiMARK (gadoversetamide)
  • MultiHance (gadobenate dimeglumine)
  • Prohance (gadoteridol)

Gadolinium Toxicity

Most cases of gadolinium toxicity appears to arise following GBCA administration, according to the journal Magnetic Resonance Imaging. Gadolinium deposition disease appears to contain clinical features seen in patients with nephrogenic systemic fibrosis (NSF), but also features not observed in that condition.

Gadolinium Found in Brain of MRI Patients: Mayo Clinic Study

A recent study conducted by researchers at the Mayo Clinic found leftover gadolinium in the brains of people who had donated their bodies to science, and had also undergone several MRIs with gadolinium contrast agents over the course of their lives. These patients were compared to cadavers left by patients who had never been exposed to GBCAs, who showed no evidence of gadolinium accumulation.

“Given that these agents are administered millions of times every year, this is an important discovery,” said study author, Laurence Eckel, M.D., radiologist, Mayo Clinic.

Gadolinium Deposition Disease Symptoms

  • Bone pain
  • Skin and subcutaneous tissue burning pain
  • “Chemo brain” or “brain fog”
  • Skin changes
  • Thickening / discoloration of the skin

Gadolinium Retention Evaluation Consortium (GREC)

Concern about the retention of gadolinium and its long-term effects on the body and brain led to the formation of the Gadolinium Retention Evaluation Consortium (GREC), according to Applied Radiology. The consortium held its first meeting in Naples, Italy, in November 2016, with the expressed goal being to foster and facilitate international collaboration by academic researchers and manufacturers of contrast agents.
