The blood thinner Xarelto (rivaroxaban) may pose an increased risk of serious bleeding compared to Pradaxa (dabigatran) in patients with an abnormal heartbeat known as atrial fibrillation (a-fib), a new study published in JAMA Internal Medicine has found.

What’s the Problem?
October 7, 2016 – Most patients with a-fib take a blood thinner drug to reduce the risk of stroke. However, although these drugs can help to prevent a stroke, they can also cause uncontrollable bleeding, which can be fatal, according to a research team led by David J. Graham, MD, MPH, from the Center for Drug Evaluation and Research at the U.S. Food & Drug Administration (FDA).
For the study, the researchers collected data from nearly 119,000 Medicare patients with a-fib treated with either Xarelto or Pradaxa from November 2011 to June 2014. They found little difference in the stroke risk among patients treated with either drug; however, there was a small yet statistically significant increase in risk of bleeding in the brain and stomach of patients who used Xarelto. Additionally, in patients 75 and older, Xarelto was associated with a small but statistically significant increased risk of death.
In recent years, Xarelto and Pradaxa have increasingly replaced warfarin, an older blood-thinner that is notoriously difficult to monitor. Which of the newer medications is safer, however, hasn’t been proven despite results of the JAMA study, according to the FDA.
“There are now several new oral anticoagulants and we don’t have a lot of studies that compare one to the other,” said Dr. Rita Redberg, a professor of cardiology at the University of California, San Francisco, who was not involved in the study. “As a patient, you are making a decision with your doctor on how these compare with warfarin, and which one is right for you.”
