Death Prompts FDA Class I Recall of Portable Oxygen Units
August 18, 2015 – According to a recall notice issued today by the U.S. Food & Drug Administration, OxyTOTE oxygen systems can ignite when mishandled or dropped, “causing an internal flash fire and the canister to burst.” Western/Scott Fetzer has received at least 2 reports of the device malfunctioning, resulting in 1 injury and 1 death.
The injury occurred when an OxyTote was dropped from 4 feet, and the employee who was killed set the device down by his side, FDA said.
The manufacturer was slated to begin a remediation program on all affected devices in February. Click here for a complete list of lot and model numbers affected by this recall.
